Replace your blue card

You need to replace your current blue or exemption card if it has been lost, stolen, damaged or if the card is incorrect.

If your blue card has expired, you will need to renew your card.

Lost or never received a card

If your card has been lost or stolen, you can apply for a replacement with a replacement blue/exemption card form.

This must be done within 14 days of the loss/theft happening.

If your application was approved but you never received it in the post, you can apply for a replacement with a replacement blue/exemption card form.

You only need to pay the fee if your postal address changed after you submitted your application and you didn’t let us know. Otherwise, you will need to pay the prescribed fee.

Problem with a card

Damaged card

If your card has been damaged, you can apply to replace a damaged card with a replacement blue/exemption card form. You can submit this form online. You also need to post us the damaged card you received. You will need to pay the prescribed fee.

Incorrect card

If the details on your card are incorrect, you can apply for a replacement with a change of name form. You can submit this form online. You also need to post us the incorrect card you received. You do not need to pay the prescribed fee if the details on your card are incorrect.

Change of name

We work with Department of Transport and Main Roads (TMR) to verify your identity and obtain the photo for your blue card. You must update your change of name with TMR before you let us know your change of name details.

You can let us know your change of name details by:

  • Using the change of name form. If you applied for a blue or exemption card before to 31 August 2020, you must use the change of name form and include a certified copy of your original TMR identity document with this form. You can submit your completed form to us by:
  • Using the online applicant portal. If you applied for a blue or exemption card using online applicant portal, you can complete your change of name details online. Your change of name details will be processed in real time using the TMR verification.

Replacement card when you have changed your name

If you want a new card with your change of name, you can indicate this on the change of name form.

If you have updated your change of name using the online applicant portal, you will need to complete the replacement card/exemption card form. You will need to send us a certified copy of your original TMR identity document with this form.

When applying for a replacement card, you will need to post us your current card and pay the prescribed fee.
