Replace a blue card

You need to replace your current blue or exemption card if it has been lost, stolen, damaged or if the card is incorrect. You may need to pay a fee for your replacement card.

If your blue card has expired, you will need to renew your card and fill in a new application form.

Lost or never received a card

If your card has been lost or stolen, you can apply for a replacement with a card/notice letter lost or stolen form. This must be done within 14 days of the loss/theft happening.

You also need to post us the original letter you received. If you no longer have your letter, you can submit your form online.

If your application was approved but you never received your card in the post, you can apply for a replacement with a card/notice letter not received form. You should wait at least 2 weeks for your card to arrive before filling in this form. You can then submit your form online.

You only need to pay the fee if your postal address changed after you submitted your application and you didn’t let us know.

Problem with a card

Damaged card

If your card has been damaged, you can apply to replace a damaged card with a card damaged form.

You need to post us the damaged card and the original letter you received. If you no longer have your letter, you can submit your form online.

Incorrect card

If the details on your card are incorrect, you can apply for a replacement with a card/notice letter correction form.

You need to post us the incorrect card and the original letter you received. If you no longer have your letter, you can submit your form online.

Change of name

If you have changed your name, you need to let us know with a change of name form.

You can choose whether you want a card in your new name. If you want a new card, you need to post us your card and the original letter you received. If you don't want a new card, you can submit your form online.
