Submit a scanned form

After you complete your blue card application form, you can scan it and send the scanned version to us using this online form.

Only 1 form will be accepted per submission. If you have more than 1 form, you will need to fill out the form on this page for each of the forms you need to submit.

Please ensure the scanned form you are submitting is:

  • valid for lodgement—download a current application form
  • completed in full and scanned as 1 document
  • saved as a PDF
  • payment receipt number is provided if applicable
  • less than 10MB in size.

Submit a scanned form

The Department of Justice (DoJ) collects your personal information when you submit a scanned form to Blue Card Services.

We collect your:

  • first, middle and family name
  • form type
  • blue card number or reference number (if known)
  • scanned application form and supporting documents
  • receipt number (if applicable)
  • email address
  • declarations and confirmations provided as part of your submission
  • preference to receive an email confirmation.

We use this information to:

  • receive and process your submission
  • verify your identity and submission details
  • assess your application
  • confirm payment and lodgement where applicable.

We will only use or share your personal information if Australian law, or a court or tribunal order, allows or requires us to.

If you do not provide your personal information, we may not be able to process your submission or provide this service.
