Submitting scanned applications
Uploading a paper application does not mean you have applied online. Processing and approval times may take longer for scanned and paper applications.

Once you have filled in your application form, upload it here to send it to us.
Note: You can register for an online account and submit your form to us. Registering for an online account means you don’t have to provide a certified copy of your Customer Reference Number or Identification.
Things you need to know
Before uploading your form, please ensure:
- the form is valid for lodgement - download a current application form
- all pages are completed and attached
- it is saved as a PDF file
- it is less than 10MB.
Only one form will be accepted per submission. If you are having trouble uploading your form, email it to us at firstname.lastname@example.org.
