Submit a scanned form
After you complete your blue card application form, you can scan it and send the scanned version to us using this online form.
Only 1 form will be accepted per submission. If you have more than 1 form, you will need to fill out the form on this page for each of the forms you need to submit.
Please ensure the scanned form you are submitting is:
- valid for lodgement—download a current application form
- completed in full and scanned as 1 document
- saved as a PDF
- payment receipt number is provided if applicable
- less than 10MB in size.
If you do not receive confirmation of successful lodgment, please try again.
Submit a scanned form
The Department of Justice (DoJ) collects your personal information when you submit a scanned form to Blue Card Services.
We collect your:
- first, middle and family name
- form type
- blue card number or reference number (if known)
- scanned application form and supporting documents
- receipt number (if applicable)
- email address
- declarations and confirmations provided as part of your submission
- preference to receive an email confirmation.
We use this information to:
- receive and process your submission
- verify your identity and submission details
- assess your application
- confirm payment and lodgement where applicable.
We will only use or share your personal information if Australian law, or a court or tribunal order, allows or requires us to.
