Disaster recovery replacement certificates

If your certificates have been lost, damaged or destroyed in a declared Queensland disaster area, you can apply to have them replaced if you:

  • live in the affected area and have been impacted
  • have had the certificate before.

If you have had certificates from another Australian state or territory which have been lost or damaged in a Queensland declared disaster area, they may also be replaced through this application process with us. This only applies for certificates needing to be replaced due to a disaster which has occurred in Queensland, it does not apply for a normal certificate application.

If your application is approved, we will send your replacement certificates for free. If your certificates are from another Australian state or territory, we will send your application on to the relevant interstate registry office. They will process your application and, if applicable, provide you with your replacement certificate.

    You will need to submit your application and proof of ID documents by email, post or in person to finalise your request.

  13. Privacy notice
    The Department of Justice and Attorney-General is collecting your personal information for the purpose of processing your certificate request under the Births, Deaths and Marriages Registration Act 2003 and Births, Deaths and Marriages Registration Regulations 2015. The information on this form may be provided to law enforcement agencies and to government and non-government agencies for verification of the data. Access to this information or a certificate may be granted to any person who has adequate reason to obtain it, or who meets the requirements of the access policy.
