Early pregnancy loss recognition certificate
Coronavirus response
To minimise exposure to COVID-19 for our customers and staff, we will not be accepting these applications in person.
If you have an application to submit, please send it to us by post only.
We offer our condolences for your loss.

If your delivery took place after 20 weeks, you will need to register the birth.
