Incorporated associations forms
On this page you'll find forms for incorporated associations. Using these forms is a requirement of the Associations Incorporation Act 1981.
Apply online to incorporate a not-for-profit association.
Otherwise, you can fill out the application to incorporate an association (Form 1) and lodge it in person or by post.
At the same time, you can lodge these applications for permission to use certain names:
- For permission to not have to use 'Incorporated' or 'Inc.' in your name, fill out the application for an exemption (Form 2).
- For permission to use a name that doesn't comply with other usual requirements, fill out the application for approval (Form 3).

Submit your annual return online.
Otherwise, we'll send you a paper form (Form 12) at the end of your financial year. Contact us if you need a new copy.
Audit or verification approval
Fill out the application for approval to conduct audit or provide verification statement (Form 21) to seek approval to have someone audit or verify your financial records.

Before applying, you must pass a special resolution at a general meeting agreeing to the change.

Before you can merge, each association must pass a special resolution at a general meeting.
Fill out all of these forms to merge with another association:
- application to incorporate amalgamated associations (Form 5)—this form is to merge 2 or more incorporated associations into a single association—fees apply
- notice to creditors about amalgamation (Form 6)—use this form to tell your creditors before you amalgamate that you're merging with another association—free
- notice of special resolution to amalgamate (Form 13)—this form is to declare that your association has passed a special resolution—free.

We recommend you contact us to discuss your options before calling a general meeting.
Then, fill out 1 of these forms:
- application to transfer into a cooperative (Form 23)—free
- application to transfer from a cooperative into an incorporated association (Form 24)—fees apply
- application to transfer into a corporation (Form 28)—associations incorporated after 1 July 1982 can use this form to change into a company or an Indigenous corporation—free
- application to transfer into a corporation from RECI Act (Form 29)—associations incorporated before 1 July 1982 can use this form to change into a company or an Indigenous corporation—free.
