Update your organisation's blue card details
You must let us know if your organisation’s details have changed so we can get in touch with you quickly when necessary.
This includes changes to your organisation’s name, address, contact details or authorised contacts, including when an organisation contact person changes their name.
You can update your organisation’s details—including your contact person—by completing this online form. If you are unable to use this online form, you can use the paper Update organisation details form and send it to us.
This form must be completed by an authorised representative of your organisation and the CEO’s declaration at the end of the form must be agreed to before you can submit the form.
In this form a:
- contact person is the person who receives correspondence relating to blue or exemption card applications, including notification of outcomes
- telephone contact is a person nominated to call us on behalf of the contact person, but this person will not receive correspondence or notification of outcomes.
Update organisation's blue card details form
The Department of Justice (DoJ) collects your personal information when you update your organisation's blue card details.
We collect your:
- organisation name and organisation ID
- organisation contact details
- details of authorised contacts and representatives
- information about changes to organisational roles or representatives
- supporting information or comments
- CEO or authorised representative details and declarations.
We use this information to:
- update and maintain your organisation's records
- verify authorised representatives
- manage your organisation's blue card obligations.
We will only use or share your personal information if Australian law, or a court or tribunal order, allows or requires us to.
