Transfer a Companion Card out of Queensland

You can transfer your Companion Card to another state in Australia if you’re moving or have already moved out of Queensland.

What you should know
When you move from Queensland to another part of Australia, you need to transfer your Companion Card details to the new state or territory. Use this service to start the transfer.
You’ll need to provide a current portrait-style photo of the cardholder. We’ll send the cardholder’s photo and details to the new state so they can set up the new Companion Card.
Who can use this service

How to transfer your Companion Card from Queensland to another state
Choose how to submit and follow the steps.
Step 1: Check you can submit online
To submit online you must be:
- able to log in with a digital ID using your email address
- the card applicant or a parent, legal guardian, spouse, family member or friend of the person with a disability, if you are applying on behalf of someone else.
Professional support workers, paid carers or other representatives acting on behalf of the person with a disability, should apply by phone to ensure the application is not linked to their personal account.
Step 2: Get a portrait photo of the cardholder

We’ll need a portrait style photo of the cardholder to include on the card. This is the person with the disability.
You can take this photo yourself, ask someone to take it for you or you can have it taken professionally.
Make sure the photo:
- is portrait
- shows your face
- shows your face and shoulders taking up most of the frame
- is in colour, clear and in focus
- is taken in front of a plain light background in a well-lit space.
Step 3: Start your online application
Complete the online form and upload the photo of the cardholder.
Your photo should be either doc, docx, pdf, png, jpg or jpeg file types.
Step 4: Receive confirmation of your application
We will confirm via email or SMS that we have received your application.
Step 5: Receive your Companion Card
Your application will be reviewed. You will receive an email or SMS advising the transfer has been completed.
The new state or territory may contact you for more information and will send the new card to the address you confirmed in the form.
