You can change the name on a Companion Card if the cardholder has legally changed their name.

What you should know
If you have legally changed your name you will need to update your card to reflect those changes.
You will need to provide evidence of your name change such as a copy of your:
- change of name certificate
- marriage certificate
- driver’s licence
- photo identification card
- Medicare card
Who can use this service

How to change the name on a Companion Card
Choose how to submit and follow the steps.
Step 1: Email us
Email companioncard@smartservice.qld.gov.au to tell us you would like to update the name on the Companion Card.
Include your full previous legal name, date of birth, new legal name and Companion Card number (if known).
Include a copy of one of the following documents (this does not need to be certified):
- change of name certificate
- marriage certificate
- driver’s licence
- photo identification Card
- Medicare card
Include a JPEG portrait photo of the cardholder to appear on the new card.
Step 2: Receive your Companion Card
Your application will be reviewed. Once approved you will receive an email or SMS confirming, and your Companion Card with your new name will be sent to you by post.

Step 2: Show evidence of name change and provide a photo
You will need to show evidence of your name change such as your:
- change of name certificate
- marriage certificate
- driver’s licence
- photo identification card
- Medicare card.
Include a JPEG portrait photo of the cardholder to appear on the new card.
Step 3: Receive your Companion Card
Your Companion Card with your new name will be sent to you by post.
How long it takes
- Processing times apply after applications are received.
- Allow 5 business days for your application to be processed.
- Applications missing information or documents take longer to process.
- Allow 3 weeks for the card to arrive in the mail.
- Allow extra time for public holidays.
