If your card is lost or damaged, or it needs a new photo, you can get a replacement card.

What you should know
Use this service to request a replacement Companion Card. You’ll need to provide a current portrait-style photo of the cardholder so we can issue a new card.
You can also use this service to update the photo on an existing card.

How to replace your Companion Card
Choose how to submit your request and follow the steps.
Step 1: Check you can submit online
To replace your card online you must be:
- able to log in with a digital ID using your email address
- the cardholder or a parent, legal guardian, spouse, family member or friend of the person with a disability, if you are acting on behalf of someone else.
Professional support workers, paid carers or other representatives acting on behalf of the person with a disability, should request a replacement by email or in person to ensure the application is not linked to their personal account.
Step 2: Get a portrait photo of the cardholder

We’ll need a portrait style photo of the cardholder to include on the card. This is the person with the disability.
You can take this photo yourself, ask someone to take it for you or you can have it taken professionally.
Make sure the photo:
- is portrait
- shows your face
- shows your face and shoulders taking up most of the frame
- is in colour, clear and in focus
- is taken in front of a plain light background in a well-lit space.
Step 3: Start your online application
Complete the online form and upload the photo of the cardholder.
Your photo should be either doc, docx, pdf, png, jpg or jpeg file types.
Step 4: Receive confirmation
We will confirm via email or SMS that we have received your application.
Step 5: Receive your Companion Card
We will send the new card to you by post. We’ll let you know once it’s on its way.
Step 1: Email us
Email companioncard@smartservice.qld.gov.au to request a replacement Companion Card.
Include your full name, date of birth and Companion Card number (if known).
Include a JPEG portrait photo of the cardholder to appear on the new card.

Visit us at one of our service centres and let us know you need a replacement Companion Card.
You’ll need to provide your:
- full name
- date of birth
- photo
- Companion Card number (if known).
Step 3: Receive your Companion Card
We will send the new card to you by post. We’ll email you to let you know once it’s on its way.
How long it takes
- Processing times apply after applications are received.
- Allow 5 business days for your application to be processed.
- Applications missing information or documents take longer to process.
- Allow another 3 weeks for the card to arrive in the mail.
- Allow extra time for public holidays.
