What you should know

Use this service to update contact details associated with a Companion Card, including:

  • residential address
  • postal address
  • email address
  • phone number
  • preferred contact method
  • nominated person contact details.

When you use this service to update your details, your details will update for your other cards and concessions. This includes:

  • Seniors Business Discount Card, Seniors Card, or Seniors Card + Go
  • Carer Business Discount Card
  • Home Energy Emergency Assistance Scheme
  • Medical Cooling and Heating Concession
  • Electricity Life Support Concession.

Who can use this service

You can update contact details connected to a Companion Card if:
you have an existing Companion Card
you are an authorised representative acting on behalf of the cardholder.
Don’t use this service if:
you want to change the name on the Companion Card
Instead, change the cardholder’s name.
you want to renew a card
Instead, renew a Companion Card.
you have moved or are moving out of Queensland
Instead, transfer your card interstate.
you are under 18
Instead, get your parent or guardian to submit a request for you.

How to update contact details

Choose how to submit a request and follow the steps.

Step 1: Check you can submit online

To submit online you must be:

  • able to log in with a digital ID using an email address
  • the cardholder or a parent, legal guardian, spouse, family member or friend of the person with a disability, if you are acting on behalf of someone else.

Professional support workers, paid carers or other representatives acting on behalf of the person with a disability, should submit by phone to ensure the request is not linked to their personal account.

Step 3: Receive confirmation of your request

We will confirm via email or SMS that we have received your request.

To update your contact details by phone call 13 QGOV (13 74 68).

How long it takes

  • We’ll aim to process your request within 3 business days.
  • We process phone request during the call.
  • Forms missing information or documents take longer to process.
  • Allow extra time for public holidays.
