You can cancel a Companion Card if the cardholder is no longer eligible or no longer needs the card.

What you should know
If your support needs have changed and you are no longer eligible for a Companion Card, you must let us know of this change and cancel the card.
If you are an authorised representative of a cardholder who is deceased, you can let us know and cancel the card.
Who can use this service

How to cancel a Companion Card
Choose how to cancel your card and follow the steps.
Step 1: Email us
Email companioncard@smartservice.qld.gov.au to tell us you would like to cancel the Companion Card.
Include the cardholder’s full name, date of birth, and card number (if known).
Step 2: Receive confirmation the card has been cancelled
We will confirm via email once the card has been cancelled.
Step 1: Phone us
Call 13 GOV (13 74 68) and ask to cancel the card.

Step 2: Receive confirmation
You will receive a printed receipt confirming the card has been cancelled.
