Proof of identity

When applying for a financial grant following a natural disaster through the Community Recovery Portal you must provide a driver licence and Medicare card. If you are unable to provide these, we accept other forms of identification (ID). Please note that you will need to call the Community Recovery Hotline on 1800 173 349 to provide other forms of identification.

For all grants you will need to provide:

  • identification that validates your name
  • another form of identification that validates your address.

If you are applying on behalf of dependants, you must also provide identification that validates your dependants.

You must provide at least 1 of the following forms of ID to validate your name:

  • Queensland driver licence
  • Medicare card
  • Australian immigration status
  • Australian or International passport
  • Bank statements
  • Birth certificate
  • Blue card
  • Community ID
  • Department of Defence ID card
  • Department of Veterans’ Affairs Health card
  • Interstate driver licence
  • Debit/credit card
  • Government employee photo ID
  • Health care card
  • Marriage certificate
  • National seniors card
  • Official letter from Centrelink or Human Services
  • Proof of age card
  • Private health card
  • Rates notice
  • Registration and driver licence renewal notice
  • Rental receipt
  • Screenshot from your MyGov page
  • Social security card
  • Statutory declaration
  • Student ID
  • Utilities account—for example, electricity, gas, water, phone bill.

You must also validate your address. The form of ID you use to validate your address can’t be the same as what you provided to validate your name.

You must provide at least 1 of the following forms of ID:

  • Driver licence
  • Bank statements
  • Home owners report by assessor
  • Official letter from Centrelink or Human Services
  • Rates notice
  • Registration and driver licence renewal notice
  • Rental receipt
  • Screenshot from your MyGov page
  • Statutory declaration
  • Utilities account—for example, electricity, gas, water, phone bill.

To validate your dependants, you must provide at least 1 of the following:

  • Medicare card
  • Health care card
  • Private health card
  • Screenshot from your MyGov page
  • Social security card.
