Replace your blue card

You may need to replace your blue card or exemption card if it has been:

  • lost
  • stolen
  • damaged
  • issued with incorrect information
  • issued in a previous name.

You can use the online applicant portal to replace your card if it is lost, stolen, or if you changed your name.

If you never received your card, or if it was damaged when it arrived, contact us for help.

Lost or stolen card

If your card is lost or stolen, you must tell us within 14 days.

You can apply for a replacement card:

If your card has been damaged, apply for a replacement card:

You must pay the replacement card fee.

Change of name

Before you update your blue card details, you must update your name with the Department of Transport and Main Roads (TMR).

You can update your name:

If you submit a paper form, you must include a certified copy of your updated TMR identity document.

If you want a replacement card showing your name, you must:

Card never received

If your card was approved but you did not receive it, you can apply for a replacement card.

You only need to pay a replacement card fee if:

  • your postal address changed after applied, and
  • you didn’t tell us.

Submit a replacement blue/exemption card form.

Card has incorrect details

If your card has incorrect information, submit a change of name form and return the incorrect card.

You do not need to pay a replacement fee if the card was issued with incorrect information.
