What you should know

A private donor conception procedure is a self-insemination using donated sperm, outside of a clinic or medical setting.

Registration of a private donor conception procedure is optional.

When details of a private procedure are added to the donor conception information register, the information is treated the same way as information provided by fertility clinics.

Only eligible applicants can apply to access the information.

Who can use this service

You can apply to register a private donor conception procedure if:
Correct. the procedure led to a birth
Correct. the procedure happened in Queensland (the person may have been born outside of Queensland)
Correct. you were involved in the procedure
Correct. everyone involved in the procedure agrees, including the
  • donor
  • person who gave birth
  • spouse (at the time of the procedure) of the person who gave birth, if they had one
Correct. any of the people involved in the procedure have died, and you are able to provide evidence of death.
Don’t use this service if:
Incorrect. any of the people involved in the procedure does not agree to adding information to the register.

Incorrect. the procedure happened outside of Queensland
Instead, apply in that state, territory or country.

How to add details of a private procedure

Choose how to submit the application and follow the steps.

Step 1: Gather your documents

Proof of identity

Each person involved in the procedure must provide 3 forms of proof of identification (ID). This must include proof of home address.

Supporting documents

If any of the people involved have died, you must include either:

  • their death certificate
  • a statutory declaration from the other people applying confirming the death.

Translating documents

If you need to provide proof of ID or supporting documents and they are not in English, you need to get them translated.

You must get copies of your ID documents and supporting documents certified by an authorised person.

  • completed application

Step 3: Submit your application

If submitting in person everyone involved in the procedure must:

  • visit in person to have their original proof of identity and supporting documents sighted, or
  • provide certified copies of proof of identity and supporting documents.

Opening hours:  Monday to Friday, 8.30am to 4.30pm.

Closed public holidays.

Bring with you:

  • your completed application
  • proof of ID and supporting documents.

How long it takes

Processing times apply to applications submitted by post.

Standard application: up to 20 business days

Urgent application: not available

Applications missing any required information or documents will take longer.

Allow extra time for public holidays and postage (if submitting by post).
