Public holiday office closure
The Registry of Births, Deaths and Marriages will be closed Monday 5 October for the King's birthday public holiday. We will reopen on Tuesday 6 October at 8.30am.
People who are donor conceived, donors and their relatives can apply to add details to the donor conception information register.

You can apply to add or update details on the donor conception information register at any time.
Queensland donor conceived people can add or update details through the online portal.
All other eligible applicants can submit a completed application form by post or in person.
What you should know
Sharing information can help people on the register:
- learn more about each other
- understand the medical history of genetic relatives
- decide if they would like to contact each other.
All information must be reviewed and approved by the Registry of Births, Deaths and Marriages before it's added to the register.
Information that does not meet requirements will not be shared.
Your information is only shared with eligible people who apply for access, and only with your consent.
You will be notified when someone accesses this information.
Contact choices
You can choose 1 of these options:
- No contact – your contact details are not disclosed.
- Open to contact – you choose an email address to share, and who to share it with.
If you select no contact, others are told they should not try to contact you.
If you change from open to contact to no contact, some people may have already seen your details. They may still contact you.
Information you can add to the register
You can choose what information you share on the register.
This may include:
- personal information
- a short bio or statement
- features such as height or eye colour
- educational background
- work or occupation
- hobbies and interests
- lifestyle information
- reasons you would like to connect with others
- medical information
- medical conditions that may affect genetic relatives
- email address
- you can choose to share your email address with some or all of your connections on the register.
You do not have to:
- add your contact details to the register
- contact anyone on the register
- agree to being contacted by anyone on the register.
If you do not want contact, you can still share other information about yourself.
Privacy tip
If you choose to contact someone through the register, you might want to create a separate email address for these communications. This can give you greater control over your privacy.

How to apply
Choose how to submit your application and follow the steps.
Step 1: Set up your myID
To use the portal, you must sign in using a myID account.
If you do not have an account already, visit:
You must electronically verify 2 Australian ID documents.
Step 2: Submit your application form
After verifying your identity, complete the online application form to access the register.
You may have to upload documents to confirm:
- any change of name (if applicable)
- your relationship to a donor or donor conceived person.

We will review it for approval.
Step 3: Get confirmation access is available
Once your application is approved, we will send you an email. Then you will be able to use the donor conception information register.

You need 3 forms of proof of identification (ID). These must include proof of home address.

You must get copies of your ID documents and supporting documents certified by an authorised person.

Once your application is approved, donor conception register information will be sent to your address by registered post.

Step 4: Receive information
Once your application is approved, donor conception register information will be sent to your address by registered post.
