Public holiday office closure
The Registry of Births, Deaths and Marriages will be closed Monday 5 October for the King's birthday public holiday. We will reopen on Tuesday 6 October at 8.30am.
People who are donor conceived, related to someone who is donor conceived, or assisted in reproduction as a donor, can apply to access information from the donor conception information register.

Queensland donor conceived people can apply through the online portal.
All other eligible applicants can submit a completed application form by post or in person.
If your application is approved, you can:
- get information you’re allowed to access from the register
- choose what information you share
- receive notifications.
What you should know
It is not always possible to find the information you are looking for. Sometimes records are missing or incomplete.
If this happens, we will tell you what information we have found and if we know why other records are not available.

How to apply
Choose how to submit your application and follow the steps.
Step 1: Set up your myID
To use the portal, you must sign in using a myID account.
If you do not have an account already, visit:
You must electronically verify 2 Australian ID documents.
Step 2: Submit your application form
After verifying your identity, complete the online application form to access the register.
You may have to upload documents to confirm:
- any change of name (if applicable)
- your relationship to a donor or donor conceived person.

We will review it for approval.
Step 3: Get confirmation access is available
Once your application is approved, we will send you an email. Then you will be able to use the donor conception information register.
Step 1: Gather your documents
Proof of identity
You need 3 forms of proof of identification (ID) including proof of home address.

Step 2: Certify copies of your documents
You must get copies of your ID documents and supporting documents certified by an authorised person.

Step 3: Fill in the application form
Download, print and complete the application by hand.

Step 5: Receive information
Once your application is approved, donor conception information will be sent to your address by registered post.
We may contact you by email or phone if we need more information to assess your application.

Submit your application in person at:
Brisbane registry customer service centre
Level 32, 180 Ann Street, Brisbane
Opening hours: Monday to Friday, 8.30am–4.30pm.
Closed public holidays.
Bring with you:
- your completed application form
- original proof of ID and supporting documents.
Step 4: Receive information
Once your application is approved, information will be sent to your address by registered post.
We may contact you by email or phone if we need more information to assess your application.
How long it takes
Processing times start when an application with all required documents has been received.
Standard application: up to 20 business days
Urgent application: not available
Applications will take longer if we need more information from you or the fertility clinic.
Allow extra time for public holidays and postage (if submitting by post).
