Proving who you are if you're under 18

If you’re under 18, you may have fewer proof of identity documents. Here are some examples you may have.

Option 1: Show us your Queensland licence or card

You can show us 1 of these—your Queensland:

It must be current or expired less than 2 years.

You can show us your physical card or use the Queensland Digital Licence app.

Option 2: Prove your name and address in Queensland

Most people only need to show 3 documents, but you may need up to 5.

Do all of these:

  1. Show us 1 from Category A
  2. Show us 1 from Category B
  3. Show us 1 more from either Category A or B
  4. If your home address in Queensland is not shown on your Category A and B documents, show us 1 more from Category C.
  5. If your name is not the same as shown on your Category A and B documents, show us 1 more from Category D.

All your documents must be originals

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

Use these documents to prove your legal name and date of birth on official records. You can provide 1 or 2 of these. If you provide 2, you’ll only need to provide 1 Category B document.

  • A full Australian birth certificate (not an an extract or a commemorative birth certificate).
    You can apply for a replacement Queensland birth certificate if you don’t have one, or apply to your home state if you weren’t born in Queensland
  • An Australian or overseas passport—current or expired less than 2 years
  • An Australian citizenship or naturalisation certificate
  • An ImmiCard—current or expired for less than 4 years

These are the documents you’re most likely to have.

All your documents must be originals

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

Use these documents to prove that you use the name shown on your Category A document/s. Your full name, or your initials and surname (family name), must match.

You can provide 1 or 2 of these. If you provide 2, you’ll only need to provide 1 from Category A.

  • A Medicare card that lists you on it
    • You need to show us your physical or digital Medicare card in person. This is not the same as giving us your details online for PrepL.
    • If you have a digital version of your Medicare card, you can show us the card by logging into the relevant app. You can’t use a screenshot or print-out from the app.
  • A photo student ID card
    • Your student ID must have a photo. You cannot use a school certificate or statement of enrolment.
    • You can also show us a digital version by logging into the relevant app. You can’t use a screenshot or print-out from the app.
  • A debit card, EFTPOS or credit card showing your name
  • A current Services Australia BasicsCard

These are the documents you’re most likely to have.

All your documents must be originals

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

We don't accept bank statements or mobile phone bills.

Show us 1 of these to prove that you live in Queensland:

  • an ATO tax assessment notice from the current or previous financial year—you should block out your tax file number
  • a Centrelink or Services Australia letter sent to you in the past 6 months
  • a letter from your employer. This must be on your employer’s letterhead and include:
    • the business registration number (ABN, ACN)
    • your full name and home address
    • the full name, daytime phone number and signature of your employer or someone authorised to sign for employer—this cannot be you
  • a Customer Identity Declaration—First Nations Person (F4168) form

If you don’t have any of these to prove your address

Instead, you can do 1 of these:

If you have officially changed your name, or your name is not the same on all of your identity documents, you must show us proof of your name change.

Show us 1 of these:

  • an Australian Change of Name certificate issued by a Registrar of Births, Deaths and Marriages
  • an Australian birth certificate (amended with or without notations) issued by a Registrar of Births, Deaths and Marriages.
