Evidence (proof) of identity for individuals

You may need to prove your identity to get a driver licence, take a practical driving test, or register a vehicle.

Option 1: Show us your Queensland licence or card

You can show us 1 of these—your Queensland:

It must be current or expired less than 2 years.

You can show us your physical card or use the Queensland Digital Licence app.

Interstate licence—you can use an interstate licence as part of proving your identity. Check Option 2 for the other documents you need.

Overseas licence—you cannot use an overseas licence to prove your identity.

Option 2: Prove your name and address in Queensland

Most people only need to show 3 documents, but you may need up to 5.

Do all of these:

  1. Show us 1 from Category A
  2. Show us 1 from Category B
  3. Show us 1 more from either Category A or B
  4. If your home address in Queensland is not shown on your Category A and B documents, show us 1 more from Category C.
  5. If your name is not the same as shown on your Category A and B documents, show us 1 more from Category D.

All your documents must be originals

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

Use these documents to prove your legal name and date of birth on official records. You can provide 1 or 2 of these. If you provide 2, you’ll only need to provide 1 Category B document.

Must be current

  • Australian birth certificate (full certificate, not an extract or commemorative certificate)
  • Bicentennial birth certificate, issued for births in 1988
  • Australian citizenship or naturalisation certificate
  • Queensland or Australian Federal Police officer photo identity card
  • A laminated Queensland 18+ card issued after 1 January 1992 in good condition.

Must be current or expired for less than 2 years

  • Australian photo driver licence (physical card only except for the Queensland Digital Licence)
  • Australian or overseas passport
  • Queensland high risk work licence
  • Convention travel document, also called a Titre de Voyage or Document de Voyage (a travel document for refugees and stateless people)
  • Document of Identity or Certificate of Identity, issued by Home Affairs or an official overseas source. If issued overseas, the document must have been used to enter Australia and issue the relevant visa.

Must be current or expired for less than 4 years

  • ImmiCard issued by the Department of Home Affairs
    • Australian Migration Status (AMS)
    • Permanent Resident Evidence (PRE)
    • Evidence of Immigration Status (EIS)

We don’t accept

  • An Electronic Travel Authority

First Nations customers

You can give us a Customer Identity Declaration—First Nations Person (F4168) form instead of a birth certificate as your Category A document. If you choose this option, you'll also need to show us 2 more documents from Category B.

All your documents must be originals

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

Use these documents to prove that you use the name shown on your Category A document/s. Your full name, or your initials and surname (family name), must match.

You can provide 1 or 2 of these. If you provide 2, you’ll only need to provide 1 from Category A.

Must be current

  • Debit or credit card with your name on it
  • Medicare card—you can be any person listed on the card.
    You may show us a digital version in the app, if you visit us in-person at a Transport and Motoring service centre.
  • Student photo ID card from an Australian educational institution or Registered Training Organisation.
    We don’t accept certificates or statements of enrolment.
You may show us a Queensland digital version, if you visit us in-person at a Transport and Main Roads service centre.
  • Centrelink or Department of Veterans' Affairs Pensioner Concession or Health Care card.
    You may show us a digital version in the app, if you visit us in-person at a Transport and Main Roads service centre.
  • Australian Defence Force (ADF) photo identity card (not civilian cards)
  • Australian firearm licence with photo
  • Australian security guard or crowd controller licence with photo
  • Services Australia BasicsCard
  • Interstate Proof of Age card or Photo Identification Card
  • Norfolk Island Regional Council Photo Identification Card.

Must be issued in the past 6 months

  • Department of Corrective Services Admin Form 22, Supporting Document to Identify a Person.

All your documents must be originals

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

We don't accept bank statements, mobile phone accounts or bills, or vehicle registration notices or certificates.

If any of your Category A or Category B documents do not show your current home address in Queensland, you need to prove that you live in Queensland.

Provide 1 of these documents.

Must be current

  • An electricity, gas, internet or phone account (not a mobile phone account)
  • A Queensland land tax valuation notice
  • A Queensland local government rates notice
  • A property purchase contract, lease or rental document, or mortgage or land ownership certificate
  • An Australian Electoral Commission letter or enrolment card.

Must be issued in the past 6 months

  • A letter from Centrelink or Services Australia
  • A health, property or travel insurance certificate
  • A letter from your private health insurer, with your membership number
  • A letter from any Queensland or federal government agency.

Must be current or issued within the last 2 years

  • An Australian Tax Office (ATO) assessment – must be current or previous financial year
  • An ATO tax file number confirmation issued in the past 2 years.

You should block out your tax file number.

If your current address is not on any of the documents we list

Instead, you can give us a letter from your employer. This must be on your employer’s letterhead and include:

  • the business registration number (ABN, ACN)
  • your full name and home address
  • the full name, daytime phone number and signature of your employer or someone authorised to sign for employer—this cannot be you even if you’re authorised to sign for your employer.

First Nations customers

Under 18 years of age

Australian Defence Force members

If you’re in Queensland on full-time permanent service with the Australian Defence Force (ADF) or the Defence Force Reserve, you can:

  • get a statement from Defence Housing Australia. It must:
  • be on their letterhead, either electronic or paper
  • show your full name
  • show the Queensland address where you live
  • include your entitlement to accommodation, which is paid out of your defence salary
  • show us a current Australian Defence Force photo identification card.

Defence Force family members

If you're an immediate family member of an ADF member, you can also choose this option. You must:

  • have a current civilian ADF identity card
  • live with an ADF member
  • be entitled to be transferred with the ADF member, at the expense of the ADF.

We cannot accept photocopies, scans or certified copies for proof of identity. We may copy them to check with the agency that gave them to you.

If your name has changed or is not the same on all your identity documents, you’ll need to show us 1 of these documents to prove the change.

Australian documents from the Registrar of Births, Deaths and Marriages

  • Australian marriage certificate (not a ceremonial certificate)
  • Australian civil partnership or relationship certificate
  • Australian change of name certificate
  • Australian birth certificate, amended with or without notations

Overseas marriage certificates

You can use an official overseas marriage certificate, if you also show us all the following:

  • either 1 Category A or 2 Category B documents in your married name
  • a marriage certificate with an official crest and registration number
  • if the certificate is not in English, then you’ll need an English translation from a translator approved by the National Accreditation Authority for Translators and Interpreters (NAATI).

Other documents

  • Divorce papers, Decree Nisi or Absolute (must show the name being reverted to)
  • Deed poll, issued before 1 February 2004.
