For licensed tattoo operators

Change your address

You must fill out the change in particulars form to tell us about your new address.

To submit the form, you must:

• pay a fee for your replacement licence
• mail the application to us within 14 days after the change.

Change of staff members

You must fill out the change in particulars form to tell us any changes of staff.

This is for any time that a staff member:

• leaves your business
• starts working for you.

You must submit the form by mail within 20 business days of the change.
