Blue card and exemption card application

You can use this form to prepare an application for a blue card (including renewals) or an exemption card if you will be working with children as a:

  • volunteer
  • paid employee
  • trainee student (undertaking a practical placement as part of your studies), or
  • business owner / self-employed person.

After you have prepared your application using this online form, you need to download and print a copy of your application. Then your employer, or a prescribed person, needs to confirm your identity. You will be given more information about completing and submitting your application on the next page.

  1. Can I apply for a blue card?

      A disqualified person must not:

      • sign a blue card application
      • apply to run a regulated child-related business
      • work in child-related employment
      • carry on child-related business.

      Registered teachers and police officers do not require a blue card when providing child-related services as part of their professional duties.

      You must provide 2 forms of identification that collectively show your full name, date of birth and signature.

      The table below outlines the accepted identification documents that you will need to apply for a blue or exemption card. This table will assist with selecting 2 appropriate forms of identification.

      A prescribed person must check and verify the 2 current, original identification documents selected and provide this confirmation on the printed application form.

      A prescribed person can be a police officer, lawyer, Justice of the Peace, or Commissioner for Declarations.
