Apply for a blue card for someone else

If someone cannot complete a blue card form themselves, someone else may be able to complete it for them.

This may apply if the person:

  • cannot make decisions for themselves
  • cannot physically complete the form
  • is a child and needs a parent, guardian or authorised representative to act for them.

Who can complete a form?

The person who completes the form depends on the applicant’s age and situation.

Adults

An adult’s form may be completed by a formal substitute signatory or an informal substitute signatory.

Formal substitute signatory

A formal substitute signatory may be:

An attorney must have the authority to make personal decisions for the applicant.

Informal substitute signatory

An informal substitute signatory may be someone who:

  • supports or assists the applicant
  • is not the applicant’s attorney, administrator or guardian.

You must be 18 or older to act as an informal substitute signatory.

Children

A parent, guardian or authorised representative may complete a form for a child.

Where possible, the children should be involved in the application process.

The child should also give their consent.

Documents you need

The documents you need depend on your relationship with the applicant.

Attorneys

Provide certified copies of:

  • identification
  • the Enduring Power of Attorney.

Guardians

Provide certified copies of:

  • identification
  • your guardianship documents.

Parents

If you apply for a child, provide proof of your relationship with the child.

This could include a:

  • Medicare card
  • birth certificate.

Informal substitute signatories

Provide:

  • proof of identification
  • confirmation that the applicant asked you to complete the form for them.

You must be 18 or older.
