Documents and information about your injuries

Victims of crime applying for financial assistance will need to provide information about their injuries.

Victim Assist may collect information about your injuries from:

• Queensland Police Service
• Queensland Health (including Queensland Ambulance Service).

We may also request one of the following from your doctor, counsellor, psychologist, social worker or domestic violence support worker:

• Victim Assist medical certificate
• Victim Assist comprehensive clinical report
• Victim Assist health practitioner examination report
• special primary victim report
• practitioner letter
• other medical record.

Sometimes you'll have to ask for the information and then provide us a copy.

We can only consider injuries and adverse impacts that were caused by violence.

Aggravation of pre-existing conditions

The health practitioner that usually treats your pre-existing condition will need to complete a comprehensive clinical report. This report will tell us how your condition has changed and what (if any) additional treatment or medical support you need.

Documents and information about your recovery expenses

You need to provide a receipt or invoice for any recovery expense that you wish to claim from Victim Assist Queensland.

For Australian and overseas providers, the receipt or invoice must include:

• what treatment or service was provided
• the cost of the treatment or service
• details of any insurance, Medicare (only applicable to Australian providers), government or any other sources of funding.
