Proof of identity
When applying for a financial grant through the Community Recovery Grants Portal following a disaster, you will be asked to provide two (2) separate forms of accepted identification (ID) – one (1) photo ID, along with another form of accepted identification type.
- If you cannot provide photo ID, three (3) accepted forms of ID must be supplied.
- One (1) form of accepted ID must be supplied for a partner and each dependant listed on the application.

If you are unable to provide these, call the Community Recovery Hotline on 1800 173 349 to provide other forms of identification.
The two (2) preferred forms of ID to apply are:
- Queensland driver licence
- Medicare card.

Evidence of address
All Community Recovery grants require evidence that your principal place of residence is the impacted address at the time of the event.
Evidence should include official documents that state your name and impacted address. See the full list of accepted Evidence of Address below.
For income tested grants, Evidence of Address is also required for a partner and each adult dependant listed on the application.
