Primary or related victims can apply online or by post using a downloaded application form.

You can include bank details on your application form. You can also provide them separately using the Notification of banking details form.
A special primary victim can ask a counsellor, doctor, psychologist or domestic violence worker to complete a special primary victim report. This form can be used to give us information about the crime and the victim’s injuries.

Victim Assist Queensland may ask for health practitioner reports about the victim’s injuries and treatment. Practitioners can download and use the following medical report forms.
The Director-General, Department of Justice (DoJ) may issue a guideline under section 131 of the Victims of Crime Assistance Act 2009.
Current technical guidelines:
The Director-General, DoJ may issue a table of costs under section 132 of the Victims of Crime Assistance Act 2009.
The table of costs outlines reasonable costs that can be claimed by a victim of violence. This is not an exhaustive list, which means costs that aren’t on the list may still be claimed.
