Victim support agencies can refer victims to the QCS Victims Register.

Who can use this service

You can refer a victim if:

  • you work for a victim support agency
  • you work for a support agency that is assisting a victim of crime.

Choose how to refer and follow the steps.

Step 1: Gather information and documents

Gather information about the victim and offender, including:

  • victim's name
  • victim's date of birth
  • victim's contact details (phone number and/or email address)
  • offender's name
  • offender's date of birth (if known).

Step 2: Proof of identity

Attach one copy of the victim’s proof of identity, either:

  • driver licence
  • passport
  • photo identification.

The documents do not need to be certified.

Step 3: Complete the form

Download, print and complete the application form.

Step 4: Victim declaration

The victim you are referring must sign the declaration.

Step 5: Submit the application

Send the complete application and supporting documents to:

Step 6: Receive the outcome

You will receive confirmation the referral has been received.

Processing time begins when a complete application is received.

Standard applications: up to 10 business days

Urgent applications: within 2 business days
