Personal information collected by the Queensland Ambulance Service (QAS) is handled in accordance with the Information Privacy Act 2009.

The QAS is collecting your personal and property access details to allow the Emergency Medical Dispatcher (EMD) to provide this information to paramedics dispatched to your property in the event of an emergency.

All personal information will be securely stored and only accessible by QAS staff. The information you provide on this form will be destroyed after 12 months. It is your responsibility to send us this information annually to keep the details on record. Your personal information will not be disclosed to other third parties.
