Access to adoption records

Birth certificate before adoption

The Registry of Births, Deaths and Marriages can provide before adoption information to the adopted person or birth parent (or eligible relative). You need to get written authorisation from Adoption Services before applying for a certificate or information from us.

Some applications may be referred to the Registrar-General for consideration.

If the adoption took place outside of Queensland, contact the registering authority interstate or overseas.

How to apply

To apply for a before adoption birth certificate:

  1. Get an authorisation from Adoption Services.
  2. Fill in the birth certificate application form.
  3. Get your proof of ID documents certified by a Justice of the Peace or another authorised person.
  4. Attach the original authorisation from Adoption Services and your certified proof of ID documents.
  5. Pay the application fee (if you did not pay online).

By post

Post your completed application form, or printed order receipt (if you paid online), with your original authorisation from Adoption Services and certified documents to:

In person

Visit the Brisbane registry counter at Level 32, 180 Ann Street, Brisbane with the completed application form, your original authorisation from Adoption Services and original proof of ID documents.

Pay the fee

The total cost is calculated when you fill in the application form online.
You can pay:

  • online
    • by credit card (Visa or MasterCard)
  • at the counter
    • by EFTPOS or credit card
    • with cash
    • with a business account cheque
    • with a bank cheque
    • with a money order
  • through the mail
    • by credit card
    • with a business account cheque
    • with a bank cheque
    • with a money order.

Make cheques and money orders payable to 'Registry of Births, Deaths and Marriages'.

We do not accept personal cheques.

Postage fees

Standard postage is included in the price when you order a certificate to be sent by mail.

You can request to have your certificate posted by express or registered post for a fee.
