How do you feel?—Patient reported outcomes survey

Queensland Health is asking patients about their health outcomes, symptoms and impact their illness or condition has on their quality of life. Your survey responses will help us better support your treatment needs and care.

Queensland Health (the department) is collecting your or your child's personal information for the purpose of better supporting your or your child's treatment needs and care.

Personal information collected by Queensland Health is handled in accordance with the Information Privacy Act 2009 (Qld).

Your or your child's personal information, including your responses to surveys (also called questionnaires), will be shared with the relevant Queensland Hospital and Health Service (HHS) and will be added to your or your child's hospital record. Your clinician will review your responses with you during your scheduled appointment. This will help your clinician understand your or your child's symptoms and plan your or your child's care.

Your or your child's personal information will not be disclosed to other third parties without consent, unless the disclosure is authorised or required by or under law.

When providing free-text responses, please do not provide personal information about any other person, for example by including their name, in your response.

The system used to survey patients is known as ZEDOC and is provided by The Clinician Limited. All information you provide will be securely stored in Australia and handled in line with the Act.

For any questions regarding this collection notice, please contact us. You can submit an enquiry form, contact by phone or the National Relay Service.

For information about how Queensland Health protects your or your child’s personal information, how to access your or your child’s personal information or how to make a complaint about a breach of the Queensland privacy principles and learn how we deal with such a complaint, please refer to our Privacy Policy.

Where did we find your or your child's name and phone number to send you a text message (SMS) survey invitation?

Personal information that is collected by Queensland Hospital and Health Services (HHS), including your or your child's name and your mobile phone number, is used to send patients or parents/carers of young patients a survey invitation. The mobile phone number on your or your child's patient record, in the hospital’s patient administration system, has been used to send the text message (SMS).

Queensland Health is able to contact patients and parents/carers of young patients by this mobile phone number for the purpose of monitoring the health service where they received care and/or treatment. Specific Queensland Health legislation (Section 150 in the Hospital and Health Boards Act 2011) allows Queensland Health to use the confidential information to contact patients for evaluating, managing, monitoring or planning health services.

Further information

Further information, including patient information sheets, is available for each survey. Taking part in the survey is optional and you can opt out of surveys at any time. You do not need to answer any question you do not want to. Your or your child’s hospital care will not be affected if you choose not to take part.

PROMs Gastrointestinal symptoms—for patients with an appointment at the Gastroenterology Outpatient Clinic

Patients who have an upcoming appointment with the Gastroenterology Outpatient Clinic at the Princess Alexandra Hospital are asked to complete an online clinical questionnaire.

The questionnaire is called the Structured Assessment of Gastrointestinal Symptoms (SAGIS). It is known as a Patient Reported Outcome Measure (PROM). It will ask questions about the severity of your symptoms and your general health in the past 7 days.

It is important you complete this questionnaire before your appointment with the Gastroenterology Outpatient Clinic. Your answers will be linked to your hospital record and your clinician will review these with you at your appointment. This will help them understand your symptoms and inform your treatment plan.

Your answers will be available to you at the end of the questionnaire. Please discuss any concerns or worries with your treating clinician at the appointment.

You may receive a text message (SMS) from QLD Health asking you to answer some questions. Click on the link in the SMS to fill in the SAGIS questionnaire. Your feedback will be secure and confidential.

The SMS will be sent to the mobile phone number listed in your hospital record. It will be sent 7 days before your scheduled appointment with the clinic. Your phone needs to be connected to the internet to complete the questionnaire. The SMS will include the name of the hospital where your appointment is. This is an example of the SMS:

The links will include characters at the end. These make the links secure and unique for you.

The questionnaire will ask about your gastrointestinal symptoms. It will take about 5 minutes to fill out.

At the end, you can provide your email address to receive future questionnaires by email.

You will be sent a questionnaire 7 days before every appointment at the Gastroenterology Outpatient Clinic at the Princess Alexandra Hospital.

You will not be sent a new survey for any follow-up or rescheduled appointments that are within 2 weeks of your last appointment.

You can find more information in our Patient information sheet (PDF, 374.3 KB).
