PREMs emergency department survey—for patients who attended an emergency department

Queensland Health is asking patients and parents/carers of young patients to take part in an online survey about their health care experience.

Most patients will receive an invitation to take part in the survey after they attend an emergency department that is participating in the survey (see Current surveys and hospitals below). For patients aged less than 18 years, we would like a parent or carer who went with the patient to the emergency department to complete the survey. You may also receive an invitation to take part in a PREMs Inpatient survey if you are admitted to hospital overnight as well as attending the emergency department.

A text message (SMS) will be sent to the mobile phone number listed in your/your child’s hospital record two (2) days after you/your child leave the emergency department. The SMS will include a link to the survey. Your phone will need to be connected to the internet to complete the survey.

If you have previously provided your email address when completing a different survey, you will be sent an invitation to this survey by email.

After clicking on the survey link, an information page will open followed by the survey page where you will be invited to take part in the survey. If you agree, there will be a short survey that will take about 5 minutes to complete. After this you will be invited to answer additional questions about other parts of your/your child’s care. These will take about 10-15 minutes to complete.

PREMs endoscopy survey—for patients who have had an endoscopy procedure

Queensland Health is asking patients and parents/carers of young patients to take part in an online survey about their health care experience.

Most patients will receive an invitation to take part in the survey after they have had an endoscopy procedure. For patients aged less than 18 years, we would like a parent or carer who went with the patient to the procedure to complete the survey.

A text message (SMS) will be sent to the mobile phone number listed in your/your child’s hospital record about seven weeks after the endoscopy procedure. The SMS will include a link to the survey. Your phone will need to be connected to the internet to complete the survey.

PREMs inpatient and out of hospital care survey—for patients who recently received care and treatment in hospital and/or out of hospital

Queensland Health is asking patients and their parents and carers to take part in an online survey about their health care experience.

Most patients will receive an invitation to take part in the survey after they have received care and treatment in hospital and/or out of hospital. For patients aged less than 18 years, we would like an adult who spent time with the patient to complete the survey.

For mothers who have recently given birth and received care at the hospital, the questions will just be about the care you received.

A text message (SMS) with a link to the survey will be sent to the mobile phone number listed in your/your child's hospital record two days after the care from the hospital ends. Your phone will need to be connected to the internet to complete the survey.

PREMs mental health inpatient Your Experience of Service (YES) survey—for consumers who recently received care from a mental health service

Queensland Health is asking consumers to take part in an online trial of the Your Experience of Service (YES) survey.

Most consumers aged 18 years or over will receive an invitation to take part in the survey after they leave a mental health service that is participating in the trial.

The online survey replaces the paper-based YES survey you may have completed before. You may still receive a paper-based survey if you have received care at another mental health service as there are only eight Queensland Health services participating in the trial (see Current surveys and hospitals below).

A text message (SMS) will be sent to the mobile phone number listed in your hospital record two days after you leave the service. The SMS will include a link to the survey. Your phone needs to be connected to the internet to complete the survey.

PREMs outpatient department survey—for patients who have recently had an outpatient clinic appointment

Queensland Health is asking patients and parents/carers of young patients to take part in an online survey about their health care experience.

Most patients will receive an invitation to take part in the survey after they have had an outpatient clinic appointment. For patients aged less than 18 years, we would like an adult who accompanied the patient to the outpatient appointment to complete the survey.

A text message (SMS) with a link to the survey will be sent three days after your/your child’s outpatient appointment. The SMS will be sent to the mobile phone number listed in your/their hospital record. Your phone will need to be connected to the internet to complete the survey.

PREMs  paediatric emergency department survey—for paediatric patients who recently had an emergency department visit at Queensland Children’s Hospital

Queensland Health is asking patients and their parents and carers to take part in an online survey about their recent health care experience.

Most parents and carers of patients who had an emergency department visit at Queensland Children’s Hospital will receive an invitation to take part in the survey.

A text message (SMS) will be sent two (2) days after your child’s visit to the Queensland Children’s Hospital emergency department. The SMS will be sent to the mobile phone number listed in their health record. Your phone will need to be connected to the internet to complete the survey.
