If you have changed your name, you need to provide us with supporting documentation, such as:
- a photocopy or scan of a marriage certificate
- your Queensland driver licence number, Centrelink customer reference number (CRN), or Commonwealth Department of Veterans' Affairs (DVA) file number

We will automatically issue you with a new card, once we have verified the changes to your name.
You can also call 13 QGOV (13 74 68) during business hours and make changes to your card over the phone.
Privacy notice
The Department of Customer Services, Open Data and Small and Family Business (CDSB) collects the information on this application form on behalf of the Department of Families, Seniors, Disability Services and Child Safety (DFSDSCS) for the purpose of assessing your eligibility for the cards or concession in question, issuing the card or concession, and to improve services and programs. Without this information your application cannot be processed.
To assist in assessing your eligibility, we may disclose your personal information to other government agencies, such as Services Australia, Centrelink, the Department of Transport and Main Roads (Qld) and the Department of Veterans’ Affairs (Cth). We will not otherwise disclose your personal information to a third party without your agreement, except in accordance with the Information Privacy Act 2009.
We may also use information in relation to other card and concession services you hold, or we may request information from other agencies (e.g. Registrar of Births, Deaths and Marriages), to update existing customer records across other card and concession services, such as the Seniors Card, Seniors Card + go, Seniors Business Discount Card, Companion Card, or other state government concession or card services.
Your personal information will be handled in accordance with the Information Privacy Act 2009 (Qld).
