    3. Are you changing your name after being married, separated or divorced? *

      You do not need to change your name with the Registry of Births, Deaths and Marriages after getting married or separated/divorced. You will need to speak to the organisations you are dealing with to find out what they require.

      We can’t change a name back to the name on your birth certificate, where both names are the same (i.e. Jane Doe to Jane Doe).

      We can only change your name if you have been living in Queensland for 12 months before your application (or more).

      • changing your record of sex

      • marriage or divorce

      • protecting yourself or close family

      • other exceptional reasons.

      1. Your name at birth These are the first names and family name on your birth certificate.
      2. Your new name If your application is approved, these are the first names and family name you will use and will appear on your certificate. You will need to change your name everywhere you do business in the community.

        2. If your previous change of name was done through a different office, we can’t change your name back to the name already shown on your birth certificate.

          If you need us to review your situation you must attach a request exception to rules form giving us a detailed reason. Exception to rules are not often approved.

          The application fee will not be refunded if your request is refused.

          You will need to show us your birth certificate. If the original birth certificate is not in English, please have it translated into English by a certified translator. Both a certified copy of your original birth certificate and the translated document must be provided.

        You have changed the type of post. We value identity protection, so we recommend that you choose registered post to ensure the delivery of your certificate can be tracked and delivered to you securely within Australia.

      10. Important update

        Australia Post are suspending their express post guarantee of next-day delivery. Express post is still available and treated as a priority, but may not be delivered the next business day every time. Normal application processing timeframes still apply.

      11. Important information

        We value identity protection and you have chosen a more secure delivery outside Australia. However, be aware that international registered mail can’t be tracked, and signing is not available in every country.
