Apprentice and trainee records
If you are an apprentice or trainee in Queensland, you can apply to the Department of Trade, Employment and Training (DTET) for replacement records.
How to apply
If you are the apprentice/trainee, complete the historical records request form.

Fees and processing
- Cost: $23.17.
- Payment: You will receive a payment request once your record is located.
- Processing time: 10 working days after payment is received.
- Financial hardship or special circumstances: Notify us when submitting your form.
Available records
The types of historical records that can be issued by DTET are outlined below:

| Trade certificate | A certificate issued when an apprenticeship or traineeship was completed in Queensland. | Submit the historical records request form to DTET. |
| Extract of service | A letter detailing the duration of time served in a trade or calling. Primarily used to apply for "time credit" when transferring or continuing a training contract interstate. It may also assist with trade licence applications. | Submit the historical records request form to DTET. |
Qualification certificate | A qualification certificate issued by a registered training organisation (RTO) (e.g. Certificate III in Carpentry). | Contact your RTO or visit Training.gov.au. If your RTO closed before 30 June 2012, submit the historical records request form to DTET. |
Statement of results | A statement of results issued by a registered training organisation (RTO). | Contact your RTO or visit Training.gov.au. If your RTO closed before 30 June 2012, submit the historical records request form to DTET. If your RTO closed after 30 June 2012, contact Australian Skills Quality Authority (ASQA). |
