Use this application form if you have already submitted an application to end your civil partnership in Queensland and want to withdraw it during the 90-day termination period.

Step 2—Complete this part by post or in person

  1. Visit a Justice of the Peace with your proof of ID documents and completed application form.
  2. Submit your application form and supporting documents to us either:
    1. by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD 4002.
    2. in person at the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane.

We can't withdraw your application

The Registrar officially terminates your civil partnership 90 days after you submit the completed application to end it.

You are not eligible

This application must be lodged by one or both of the registered partners within the 90-day termination period.
