Fill in a withdraw from registering a civil partnership application form

Use this application form to withdraw your submitted application to register a civil partnership without a declaration ceremony. Our office must receive this application form within the 10-day cooling-off period.

Step 1—Complete this part online

  1. Start the form by answering the question below—it should take you about 15 minutes to complete this.
  2. Download and print the completed application form.

Step 2—Complete on paper and submit

  1. Have another adult (18 or older) witness your signature(s) in the declaration section of the application form.
  2. Gather your proof of ID and supporting documents.
  3. Proof of ID documents must be submitted by post or in person. When submitting by post, have your documents certified by a Justice of the Peace or another authorised person before sending.
  4. Submit your application form and supporting documents to us either:
    1. by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD 4002
    2. in person at the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane, or one of our agents at a Queensland Magistrates Court or Queensland Government Agent Program (QGAP) office (excluding the Brisbane Magistrates Court).

We can only withdraw an application to register your civil partnership that has been lodged in the last 10 days.

You will need to apply to end a civil partnership.

You are not eligible

This application can only be lodged by one or both proposed civil partners who have applied to register their civil partnership. The withdrawal of the application must be received by our office within the 10-day cooling-off period.

You need to prove your identity
