Fill in a declaration of civil partnership form

Civil partnership notaries can use this form to notify us when you have conducted a declaration ceremony for a couple in Queensland.

Step 1—Complete this part online

  1. Start by answering the questions below—it should take you less than 15 minutes.
  2. Download and print the completed declaration form before the declaration ceremony.

Step 2—Complete on paper and submit

  1. Make sure that all of the declaration sections are signed.
  2. Submit the signed declaration form either:
    1. by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST  QLD  4002
    2. in person at the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane.

Remember, the information you give needs to be the same as the details the couple gave on the notice of intention to enter a civil partnership.
