We have emailed you the completed application form, along with instructions for what you need to do next.

How to submit your application

To receive an eligible certificate:

1. Read the email we send you to find out how to finalise your application.
2. Print and sign your application.
3. Have an authorised person (if submitting by post) certify your:
   • proof of ID
   • supporting documents for certificate fee waiver requests.
4. Submit your signed application, proof of ID, evidence of financial hardship and immediate need by email, by post or in person.

What next

We will review your application and a registry officer will only contact you if we need additional supporting documents.
