Queensland commemorative birth certificates

Commemorative birth certificates are not official proof of identity documents and you may buy one online for you or your child.

If you want to buy a commemorative birth certificate package that includes a standard birth certificate or just a standard birth certificate for official purposes (e.g. applying for a driver licence) you can use our online application form and submit proof of ID by post or in person.

Access to this information or a certificate may be granted to any person who has adequate reason to obtain it, or who meets the requirements of the access policy.
