Use this application form to buy a commemorative birth certificate for a birth or adoption that took place in Queensland.

To submit your application form:

  1. Answer the questions below—it should take you less than 5 minutes.
  2. Pay $27.30 by MasterCard or Visa.


All Queensland commemorative birth certificates fit into a standard A4 picture frame (certificate size: 210mm x 297mm).

  4. Commemorative birth certificates can only be issued for people born or adopted in Queensland. They can't be used for official purposes—only an official birth certificate can be used for identification purposes.

The Department of Justice and Attorney-General is collecting your personal information to process your application for birth certificates under the Births, Deaths and Marriages Registration Act 2003 and Surrogacy Act 2010. The information on this form may be provided to law enforcement agencies and to government and non-government agencies to verify the data. Access to this information or a certificate may be granted to any person who has adequate reason to obtain it, or who meets the requirements of the access policy.
