Public holiday office closure
The Registry of Births, Deaths and Marriages will be closed Wednesday 12 August for the Royal Queensland Show (Ekka). We will re-open on Thursday 13 August at 8.30am.
You can recognise your loss with a commemorative certificate in Queensland. Apply to the Registry of Births, Deaths and Marriages.

When you apply for a certificate, you need to:
- have the treating medical practitioner or midwife sign the application form
- choose a design
- decide what information you provide for the certificate.
Who can use this service
| You can get an early pregnancy loss certificate if: |

| Correct.you experienced the loss of your baby—delivered before 20 weeks' gestation and weighed less than 400g in Queensland. |
| Correct.you are the parent who experienced the loss. |
| Don’t use this service if: |

| Incorrect. your baby was born after 20 weeks or weighed more than 400g Instead, register your child's birth. |

Download, print and complete the form by hand.
Step 2: Get a health professional's signature
Have your treating medical practitioner or midwife sign the declaration section on the form.
Step 3: Submit your application

Step 4: Receive the certificate
Once your application is approved we will post the certificate to you.
Your application may not be approved if you:
- don't meet the rules
- don't include the right documents.

Submit your application in person at:
Brisbane registry customer service centre
Level 32, 180 Ann Street, Brisbane
Opening hours: Monday to Friday, 8.30am–4.30pm.
Closed public holidays.
Bring with you:
- completed application form.
Step 4: Receive the certificate
Once your application is approved we will post the certificate to you.
Your application may not be approved if you:
- don't meet the rules
- don't include the right documents.
How long it takes
Processing times start when an application with all required documents has been received.
Standard application: up to 10 business days
Urgent application: not available
Allow extra time for public holidays and postage.
