Fill in a note a change of name in a birth or adoption register application form

Use this application form to note a change of name if you were born or adopted in Queensland and have changed your name:

  • in another state
  • through another legal process, such as by deed poll.

Step 1—Complete this part online

Step 2—Complete on paper and submit

  1. Have another adult (18 or older) witness your signature in the declaration section.
  2. Gather your proof of ID and supporting documents.
    Proof of ID documents must be submitted. When submitting by post, get your documents certified by a Justice of the Peace or another authorised person.
  3. Submit the application form along with payment and supporting documents, to us either:
    1. by post to Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST  QLD  4002
    2. in person at the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane.
