Fill in a marriage certificate application form

Use this application form if you or the person you are applying for were married (and the marriage registered) in Queensland.

Step 2—Complete this part by post or in person

  1. Get your proof of ID documents.
  2. Submit your proof of ID in person or by post.
Proof of ID documents must be submitted by post or in person. Get your documents certified by a Justice of the Peace or another authorised person, if submitting by post.

The Registry of Births, Deaths and Marriages located at Level 32, 180 Ann Street, Brisbane will be closed Monday 25 December 2017 for the Christmas break. We will re-open on Tuesday 2 January 2018 at 8.00am.

Get it right

Your application will take longer to process if your documents are not correct.

Before submitting your application remember to:

  • get your proof of ID and supporting documentation certified if you are posting your application.
  • allow extra time for Australia Post.
