Complete this form to request copies of materials which are held by us. Depending on what you request, a fee may apply. We'll get in touch to let you know how much the fee is.

Please be as detailed as possible when providing us info about the records you need.

      Deed poll records can only be supplied to the person a record relates to.

      If you require records for another person, you must apply to the supreme court. Queensland State Archives will be unable to assist you.

      If you are requesting your own records, please give us the following information:

      • • Full names prior to and after the change.
      • • Any name changes that also occured through marriage or divorce.
      • • The approximate date (within a 5 year range).
      • • The approximate region in which the change was made.

      What type of records are you looking for? Examples include Wills, Court cases, Immigration records etc.

      When requesting information about family records held by us, please supply the following:

      • • The type of records are you looking for
      • • Full names of the people involved.
      • • A geographical place or location.
      • • An approximate date (within a 5 year range).
  5. Legal or court record requests (If you chose ‘Legal or court records’ above)

      Please note, many court records are held under a Restricted Access Period and may be closed to the public.

      When requesting information about court records held by us, please supply the following:

      • • What are the names of the people involved?
      • • What is the name of the court the case was heard in?
      • • What was the activity heard before the court (ie criminal or civil?)?
      • • What was the date (within 5 year time range)?

      To request information about land records held by us, please supply the following:

      • • What type of land record are you searching for?
      • • In which Parish and County was the land located?
      • • Is there a person's name associated with the land?
      • • What was the date (within 5 year time range)?
      • • If known, what was the Land Agent's district?

      Please note, many records are held under a Restricted Access Period and may be closed to the public. Please allow sufficient time to contact the responsible agency for permission to use the closed records in your research.

      When requesting records held by us, please supply the following:

      • • What is the topic or activity you are researching?
      • • Do you have a deadline for your research and what is it?
      • • What was the activity heard before the court (ie criminal or civil?)?
      • • Full names of the people involved.
      • • A geographical place or location.
      • • An approximate date (within a 5 year range).

      • • The name of the school
      • • Student names
      • • Student admission dates
      • • Student attendance period or years
      • • Teacher names
      • • Teacher's service period at the school, within a 5 year range.

      Please provide the following when requesting Transport History records held by us:

      • • The type of records are you looking for (for example railway or ship records)
      • • If the records are about railway carriages (rolling stock) or railway station plans
      • • Any name or unique identifiers of the rolling stock or ship
      • • The name of the station or port
      • • A geographical place or location
      • • An approximate date (within a 5 year range).

      Please be aware that many records are held under Restricted Access Periods, and may be closed to the public. We may need time to contact responsible agencies if their permission is required to share closed records.
