Charity and fundraising forms

Use the following forms for various actions relating to managing a charity.

These forms are required as per the Collections Act 1966.

Setting up a charity

Application for registration of a charity (Form 1)
Complete and submit this form to register a charity or apply online—select Fair Trading Application for Associations and Non-profits.
If you are registered with the Australian Charities and Not-for-profit Commission (ACNC), you do not need to apply for registration. Complete and submit an ACNC registration notification instead.
No fee applies.
Application for a sanction (Form 5)
Complete and submit this form to get a sanction (approval) to fundraise for a community purpose or for a one-off appeal, or apply online—select Fair Trading Application for Associations and Non-profits.
No fee applies.
ACNC registration notification (Form 23B)
If your organisation is registered with the ACNC and you wish to fundraise in Queensland, apply online—select Fair Trading Charity ACNC Registration Notification.
If you are registered with the ACNC, you don’t need to apply to register the charity with us.
No fee applies.

Application for removal of a charity (Form 4)
Complete and submit this form if you believe a particular existing charity should be deregistered.
No fee applies.

Authority to collect for an association (Form 7)
Complete and submit this form to get permission for door-to-door and street collectors to accept donations on behalf of a charity or sanctioned association.
No fee applies.
Application to have a day assigned for a door-to-door appeal or street collection (Form 8)
Complete and submit this form to apply to us to have a day assigned for door-to-door or street collections.
No fee applies.
