The forms below support the application, allocation, property and tenancy management processes established under allocations policies and procedures.
- Form for people who want to apply for public housing.
- Providers funded under the long term community housing or affordable housing program must complete this form to give new or updated property details to the department.
- Providers use this form to nominate applicants to the department for transitional housing or approved long term community housing properties. This form must be used together with the Notification of Vacancy Form (below).
- Housing providers must use this form to notify the department when a vacancy becomes available.
- Providers must use this form to inform the department of any offers of public housing made to applicants, or exits from transitional housing.
