Existing residential service providers

A service provider is the operator or owner of a registered residential service. A provider can be:

  • an individual or individuals
  • a corporation

Change a service provider

To change a service provider, you need to:

  • complete an application to become a registered service provider of a currently registered residential service (change of service provider)Residential Services Form 4 (PDF, 206KB)
    • pay the application fee of $133.25
    • pay the criminal history check fee of $35.90 (for each person applying)
    • attach a certified copy of proof of identification (i.e. a birth certificate, drivers licence, passport) for each provider and associate involved with the residential service.

If the residential service is run by a partnership or group of people, ensure each partner or individual provides their details on the application.

Associates

An associate may be an executive officer of a corporation who manages a residential service. An associate can also be an individual who manages the service on behalf of the service provider and makes business decisions on their behalf. You should notify us as soon as possible of any changes in relation to an associate managing your residential service.

Changes

If you are adding or changing an associate as well as the service provider, complete an application to become a registered service provider of a currently registered residential service (change of service provider)Residential Services Form 4 (PDF, 206KB)

If you are only adding/changing an associate:

  • complete a Notification of commencement of an associateResidential Services Form 6 (PDF, 173KB) 
  • pay the criminal history check fee of $35.90 (for each person applying) 
  • attach a certified copy of proof of identification (i.e. a birth certificate, drivers licence, passport) for each provider and associate involved with the residential service. 

Sending certified documents

Take the original document and a photocopy to a Justice of the Peace, Commissioner for Declarations, solicitor or barrister. They will stamp and/or sign the photocopy to make it a certified copy for your application.

Processing time

Applications for registration will be processed in 4–6 weeks. It may take longer if we need to contact you for missing information or fees.
