Requesting old or replacement records and documents

Apply for a replacement of old or lost records and documents by completing a form on the Apprenticeships Info website.

You can ask for a/an:

  • certified copy of completion certificate or extract noting completion (this certificate is separate from your qualification certificate which is issued by your training organisation or college)
  • certified copy of indentures (issued for apprenticeships started before the early 1980s)
  • extract from the register of apprenticeships and traineeships (for those who completed partial training in Queensland and want credit to continue their apprenticeship or traineeship in another state or territory).

There is a fee from $16.30 to $17.60.

You will be contacted within 10 working days to discuss the records found, payment options and where to send the documents. If the document is unavailable, you can discuss other options.

Documents older than 65 years are handled by Queensland State Archives.
