Ongoing support and planning

Your family, health professionals and service providers form a team. Good teamwork needs good communication. Just as a sporting team has a game plan to achieve its winning goal, you have plans and dreams for your child.

Formalised planning helps services make the most of their resources for your child or family's benefit. Planning isn't static—it's ongoing. And it can change with your child or family's needs.

You'll find that ‘one size does not fit all'. The support you need may not come from one organisation or service. Encourage your team to think creatively, putting together a package of different services—perhaps requiring a number of plans—to achieve the support that's right for your family.
