Performance reporting for Community Care
In addition to Minimum Data Set and financial reporting requirements, some organisations funded to provide Community Care services must also complete the following performance reports.
Schedule 2D
Any organisations funded to deliver information, education and training (Service Group 8) must complete a Schedule 2D report.
Schedule 2E
Any organisation funded as a Community Access Point must submit as per the reporting requirements outlined on the Schedule 2E–Access Point Performance Framework.
Schedule 3
Any organisation that exceeds the variation in outputs allowed under the Service Agreement (see below) must complete the Schedule 3 Performance Issues report:
Within a single financial year, service providers:
- must deliver no less than 70% of the outputs for each funded service type*
- must deliver no less than 95% of the outputs for each funded service group
- may transfer 5% of outputs across service groups where there is at least one service type funded, with the exception of Service Group 5.
*If an Organisation is funded for a single service type, then delivery must be no less than 95% of the outputs. More information can be found in the Manual for Queensland Community Care Services:
Submission timeframes
Where applicable, performance reports must be submitted by the following dates:
Funding amount $50,000 and over
- For period of 1 July–31 December, the due date is 31 January
- For period of 1 January–30 June, the due date is 31 July
Funding amount less than $50,000
- For period of 1 July–30 June, the due date is 31 July
