Ongoing Needs Identification (ONI) tool

The Ongoing Needs Identification screening tool (ONI) provides a consistent way for Community Care providers and other community services and health officers to assess the level and complexity of a person’s support needs, as well their eligibility to receive service. The ONI assessment may be completed over the phone or in person.
